Ideal System and Continuum of
Care
Rarely can one agency provide all the services needed to meet
the multitude of problems experienced by clients. Use the population you
previously chose in the first discussion in Unit 3 (see list below) for
this discussion:
Older Adults.
Part I: Follow these steps with this population in mind:
1.Construct an "ideal system of integrated care" to adequately address the unique and multiple needs of this population in your own community.
2.Consider the role of case management in substance-abuse treatment and how this impacts continuum of care for clients. (Generate an outside, peer-reviewed journal article for cognitive behavioral therapy with substance abusers, if you choose this approach.)
3.Identify what local resource agencies (for example, Health Care for the Homeless or a local AIDS resource organization) would be important to incorporate into this integrated model.

College students can have multiple needs that face. Sometimes getting funds to attend school or even having funds while at school can be a struggle. If students don't stay on campus having transportation can also can be a problem. Peer pressure, and stress can occur and it may cause some drug activity. Vaping is also a high especially on the younger generation. As a case manager working with a college student they can assists with there need on and off campus, and put them in contact with resources and off campus. Local churches, SNAP, school counselors, advisors, The Turning point, Hazel Pittman, etc.

Focusing on youth and adolescents/ co- occurring substance abuse my plan for them would include a general building with mental health, AOD, dss, djj, dhec all in one. I picked these four resources based on my current location because these four agencies are always interwibed with the youth within the community. Not only does this help the youth, but this also helps the client.
Substance abuse treatment counselor approach will be CBT. The case manager has to ensure that they provide quality treatment modalities for the patient and acknowledge that each treatment option will not always be useful for every client. in some cases, a counselor will have to change up their techniques on treatment. A recent meta-analysis of 34 cognitive–behavioural treatment trials for illicit substance use disorders found CBT had significantly better outcomes (31% abstinent) compared with control conditions (13%), with an aggregate moderate effect size of 0.45 at post-treatment (Hides, Samet, & Lubman, 2010).

The attainment of an optimal
system of integrated care for older adults grappling with substance abuse issues
within the community calls for the adoption of a comprehensive and
collaborative approach, which commences with meticulous and systematic
assessment and screening procedures. It is imperative to conduct regular
screenings for substance abuse across diverse healthcare settings commonly
frequented by older adults, including primary care clinics, geriatric clinics,
and senior centers (Kuerbis, 2020). By engaging a diverse array of
professionals encompassing healthcare providers, social workers, and addiction
specialists, this interdisciplinary approach possesses the potential to
effectively identify and address substance abuse concerns at an early stage. It
is of utmost importance that these screening processes be meticulously tailored
to account for the unique needs and intricacies encountered by older adults,
thus ensuring their appropriateness and sensitivity.
