The intake report helps you to
gather a broad range of information about the client. Typically conducted
during your first appointment, the data from the Benchmark Benchmark Initial
Interview Assignment will help you decide appropriate treatment plans and, if
necessary, test selection. Therefore, it is important that you gather
sufficient background information in the initial interview. Typically, this
information will include the client's family and social/relationship history,
work experiences, educational history, and medical and legal histories. The
counselor is also interested in a client’s self-perceptions, significant life
experiences, and goals or aspirations.

For this Benchmark Initial Interview Assignment, the client is coming to you for one of three referral questions. You will develop a fictional character that you will interview. Your Benchmark Initial Interview Assignment should be structured according to the Benchmark Initial Interview Template.
1.Would the client be a good candidate for participation in a summer mission’s trip in a very challenging environment?
2.Would the client be a good candidate for Senior Pastor at a large urban church?
3.Would the client make a good Resident Assistant (RA) at Liberty University
Important points regarding the Benchmark Initial Interview Assignment:
1. Your character must be an adult male or female over the age of 18. You must give your “client” a name.
2.Because this psychological evaluation will not be performed for clinical, forensic, or legal reasons, your character must not have a life-threatening medical condition, a chronic or debilitating psychological disorder, or an extensive criminal history.
Format of the Benchmark Initial Interview Assignment:
1.Content: Complete all the information under the
headings of your report according to the format below. Identifying information can
be in bullet points, but use paragraphs (at least 3-4 sentences) and general
APA format for the rest of the report.
2.Write in the third person (e.g., “Mr. Jones is
a 42 years old…,” “His greatest strengths are...”)
3. Be sure to refer to the Initial Interview when completing the Mental Status Exam and Psychological Report later in the course.
Note about making up a character: Because you will complete all the psychological tests as your character for the Psychological Report, you must know this individual very well. Spend some time thinking about your character and make sure you have a clear image of who he or she is. Make a genuine attempt to visualize this person and to get yourself into the mind of the character you have created. How does this person see himself or herself? How does this person approach life, deal with challenges, and interact with others? Before you complete each of the psychological tests for the next assignment, be sure to review this information so your responses will be reasonably consistent across tests.
Referral Question Information
You must use one of the referral questions provided below as the “Reason for the Referral” portion of the report (you will answer the referral question in another report).
Referral Questions:
1.Would this examinee be a good candidate for participation in a summer mission’s trip in a very challenging environment?
2.Would this examinee be a good candidate for Senior Pastor at a large urban church?
3.Would this examinee make a good Resident Assistant (RA) at Liberty University?
Remember that this is a referral, which means that a mission’s board, a church, or Liberty University is sending the client to you for an evaluation. This would suggest that the client was vetted and appeared to be a viable candidate for one of the above positions. The referring organization would not pay to have you evaluate someone who is clearly impaired or unsuitable for the position. This is why it is very important that the person you select does not have a life-threatening medical condition, a chronic or debilitating psychological disorder, or an extensive criminal history. Also, make sure that the client’s life circumstances match the job. For example, a mother with three young children would not apply for an RA position at Liberty. Points will be deducted if your client has any of these conditions or is clearly unsuitable for the position.
* Note: Typically, in addition to gathering biographical information, the evaluator uses the initial interview to gather information using the Mental Status Exam (MSE). However, you will focus on the MSE in another assignment.
Be sure to review the criteria on the Benchmark Initial Interview Grading Rubric before beginning this Benchmark Initial Interview Assignment.

Identifying Information
The client name is Joe Allen and he lives 175 Race Course Road Avenue,Philedophia.
He is 37 years old and he is not married. He is currently working as a manager.
The date of interview is 4th November 6, 2022.
Reason for Referral
Joe has been recommended by the General Counsel
Missionary Board of the Philadelphia Church of God to accompany a group of
young people on a journey to South America to help develop schools. He is
scheduled to appear here for further examination.
Current Situation and
Functioning
Joe like mornings when he can take his time getting
ready and making breakfast. He likes to get an early start on the day, so he
often goes to the office around 6:30 am and then takes a sleep until the rest
of the staff arrives at 9:00. After work, he takes improv acting classes and
winds up his day with movie nights. Joe can handle his responsibilities at
home as well as in the workplace, but he frequently offends his coworkers
because he fails to recognize social cues.
Joe seems to show some essential ADL abilities,
despite the fact that there are certain areas of concern. He demonstrated a
high level of competence in activities such as traveling, house cleaning,
toileting dry, and tidying up. However, Joe is deficient in ADLs
skills such as personal hygiene, transportation, shopping, cooking,
and maintaining a tidy and pleasant home. Joe claims he takes less than 30
seconds on brushing and cleaning his mouth. He has demonstrated that he is
capable of getting himself from and to work on his own. Unfortunately, he is
easily distracted, and this has already resulted in a minor injury to a
colleague in the parking lot. He said his diet is "not too horrible,"
although he regularly eats fast food. When he's on the go, he'll often make
more than three trips through the drive-through every day. Joe can clean his
home well, but he lacks the practical skills to maintain it. He therefore
lives in an apartment, and the landlord of that flat can accommodate such
needs.
Relevant Medical History
There is nothing in Joe's medical history that is
pertinent to this trip that would prevent him from participating in it. However,
the fact that he spent an excessive amount of time sitting at his desk and did
not engage in any kind of physical activity contributed to his high blood
pressure. Given that Joe's living status is rather typical, he is in a position
to be able to pay for her medical care. There is no single documented instance
of Joe ever using any kind of drug.
Psychiatric Treatment History
An emotional outburst that Joe had at work resulted
in him being required to attend therapy for a period of six hours.
Family History
Joe has two brothers and sisters. He was brought
up by his mother in a family in which his parents had divorced when he was
young. Joe has a profound resentment against his stepfather due to the fact
that his mother remarried while he was a teenager. Joe's mother has been
suffering from a chronic kidney condition throughout her life. She had
already had a number of therapies in an effort to reduce the symptoms of her
kidney ailment before she was given the recommendation to undergo dialysis
therapy in order to slow the progression of her kidney impairment. At the
beginning of this year, Joe was given a diagnosis of hypertension (high blood
pressure). Following the development of an increased need to drink water and
urinate, as well as the subsequent loss of weight, he made the decision to go
to the hospital. She was found to have high blood pressure after an A1C test
was carried out on her. However, it was not severe up to a point where it could
prevent him from his daily work activities. He managed to control the condition
and now it is less concerning.
Social and Developmental
History
There are various developmental milestones
associated with Joe's understanding of his parents. He does not recall the
specifics of the events that led up to his parents' separation, but he does
remembers the emotions of grief and confusion that he had when his father moved
out. When he was in his teenage years, his mother remarried, which was another
significant event in his development. He reflected on an emotional breakdown
that occurred to him when he was at the wedding. Jeo Gary has not been in any intimate
relationships to this point in his life. He said that one of the things that
has prevented him from seeking romantic relationships is his concern that he
would be turned down by the women he meets. He forms strong bonds with those
who make an effort to be his close friend, and he has referred to more
than one person as his best friend. He has the mindset that he is a member
of the families of his classmates. He responds in a contradictory manner to
those in power. He enjoys acting independently, but at the same time, he craves
the attention and acclaim of people who are in positions of authority over him.
Educational and Occupational History
The highest level of schooling that Joe has achieved
is a high school. He enrolled in Temple University for his first year of
college, but he was unable to continue his education after that because he ran
out of money. During the following years, he had a variety of occupations to
make ends meet. Before taking over as manager of the office, he was mentored as
a salesman by the previous manager. Since 2008, Joe has been employed
by the same employer throughout his whole career. He has never served in
the armed forces. His performance at work has been met with a variety of
reactions. His immediate supervisor gave him a terrible performance assessment,
yet his office has consistently generated higher figures than the other offices
in his organization. Despite this, his supervisor gave him a negative rating.
He has not been absent from work for any reason, nor has he had any injuries
while on the job.
Cultural Influences
Joe does
not come from a community or place that is very varied. Instead, his effect on
culture has come from the entertainment industry. He is a huge fan of
television series and movies from the 2000s. He often imagines himself to be a
protagonist from one of them, and he anticipates that his life will unfold in a
manner that is analogous to the way the plot of the film plays out.
