Associate Success Role Our ACO Success Associate is a key position between our
clinical and operations departments and serves as the liaison/account manager
to help our physicians be successful in the ACO. In this project, you will
focus on your approach to how you would partner with a primary care stakeholder
to build trust for an ongoing partnership. Market and Partnership Expansion
Goals Background We have partnered with Dr. Smith Primary care in New Jersey.
Dr. Smith has 500 Medicare patients who will become attributed to Accountable
Care Organization. This means ACO assumes responsibility for the cost and
quality of Dr. Smith’s patients’ care in the eyes of Medicare. If we
effectively lower the cost of care and improve health outcomes, Medicare will
share a percentage of savings with our organization – and thus, our physician
partners. Our team of Medical Directors, RN Care Managers, Social
Workers, Nurse Practitioners and ACO Success Associates will be working
closely with Dr. Smith and his front desk staff to help provide high-touch,
preventative care for his Medicare patients going forward.
For example, leveraging data analytics to identify care gaps and ensuring
everyone gets required screenings. Scenario As the ACO Success Associate you
will be the liaison Dr. Smith’s medical practice and our Clinical team in
determining the types of interventions and clinical programs we offer to
specific patients. To do this, our team will need to obtain EMR access to see
clinical data on Dr. Smith’s patients, which Dr. Smith will need to grant to
our team. This requires Dr. Smith to spend time with his EMR vendor (such as
Athenahealth), to provide access to critical and private data. In
providing access, he is trusting us to be good stewards of his patient’s
sensitive medical information, which makes him hesitant to give us access.
Your Task 1. Please prepare a three (3) slide
PowerPoint presentation (saved as a PDF) and present it to our team during your
next panel interview to demonstrate how you would gain Dr. Smith’s trust to
convince him to grant us access to his EMR.
2. In this presentation, provide talking points, and craft a script/visual to how you would position the value of the benefit for both stakeholders (Dr. Smiths Practice/Vytalize/Patients) in obtaining access to his patients’ clinical data.
u Where ACOs
are mandated to spur the trust of primary care practices and consumers,
including patients; an ACO Success Associate is more critically involved in
liaising with all parties involved and building the necessary bridges of trust
between stakeholders.
u In
this case, as the ACO Success Associate for Vytalize Health Organization, the
development of Dr. Smith’s 500 patients in need of Medicare will demand skilled
and intentional actions that will strengthen the patient-provider relationship
and build Dr. Smith’s trust for the ACO.
u More
so, since the organization is well equipped in digital health to dedicate more
efficient ways of care for each patient, trust is essential for Dr. Smith to
share such critical patient data.
u Therefore,
two major questions have to be answered in this scenario:
1. How to
gain trust for Dr. Smith to be willing to share the necessary and critical
electronic medical records.
2. What
are the major talking points to ensure benefits value for all stakeholders?
