PURPOSE:
The purpose of this assignment is to explore Consumer Health Informatics (CHI).
CHI examines patient information from many different points of view such as:
health literacy, consumer knowledge, and education. The goal is to empower
patients while giving them the knowledge they need to make their own health
decisions. Consumer health informatics couples the consumers’ needs for
information with their healthcare preferences to create a tailor-made health
care experience. AUDIENCE: The focus of CHI is on the patient/consumer. The
audience for this assignment must be either patient or family/significant
others. Use your selected policy issue as it relates to your specific patient/
family population and health issue as your specific focus for your infographic
message. Healthcare providers or other members of the healthcare team are NOT
your audience. CONSUMER HEALTH INFORMATICS ARE: Tools or systems promotes
interactions with consumers and their providers Any tool into which a patient
can share their health information and receive a body of health information
Resources that promotes shared decision making and co-designing of care A tool
or system where information or other benefits may be used with the assistance
of a healthcare professional, but doesn’t necessarily depend on a healthcare
professional. Resources that promote patient engagement EXAMPLES OF CHI TOOLS:
Patient Portals Telehealth mHealth Apps Secure Messaging/Texting REVELANT

INSTRUCTIONS Create an infographic on how CHI can engage and empower patients, build stronger partnerships with their providers, and improve their health outcomes. The infographic does not have to tie to your policy topic. It must engage the partnership with providers to benefit the consumer's health. Submit a separate reference page in a word document in APA format to the assignment link. DO NOT include citations in your infographic. It is distracting and takes up valuable content space.
